In the Shortcuts app, open the "Automation" tab and make sure the "When LockBoard is closed" automation is set to Run Immediately.
A transient iOS glitch that can appear when the shortcut runs. Open LockBoard once, then run it again; if that doesn't help, restart your iPhone (this fixes it in most cases). If it still appears, delete the "Get Note Wallpaper" action inside the shortcut and add it back.
